  • Owner's Name
  • Owner's Address
  • Owner's After-Hours Telephone Number
  • Does the structure have burglar alarm?
  • Does the structure have a hold-up or robbery alarm?
  • Name and Telephone Number of the burglar alarm company
  • Does the structure have a fire alarm?
  • Name and Telephone Number of the fire monitoring company
  • Does the structure have a fire sprinkler?
  • What company services the sprinkler system?
  • Is there an AED (Automatic External Defibulator) on the premises?
  • Are there hazardous materials on the premises?
  • If yes, please list hazardous materials and their location.
  • How many elevators are on the property?

Also list all persons who are emergency contacts for your business or church including their name, address, after hours telephone number, their position with the business, and if they have a key to the structure.
